I totally agree on Tijuana. My buddy and I were essentially strip searched on the street for no apparent reason. I guess the cops there often do this to Americans and you need to pay them off if you don't want to go to jail. It is unbelievable what America allows Mexico to be, but that is another conversation for another day.
My favorite international city has to be Madrid. It is a good mix of Paris and Rome without so much of the hoopla from the tourists.
London, Paris, Montreal, and Edinburgh would also be somewhere at 'A' level for me, as well. Any major northeastern U.S. city also gets a thumbs-up and I am lucky enough to be able to visit NY, DC, and Boston multiple times throughout the year.

Chicago A- - had a great time there. wouldn't mind going back
New York A- - anyone that can't enjoy themselves there doesn't know how to have fun
Boston B - plenty to do there to keep entertained.
Montreal B - really enjoyed myself there on a couple of trips
Ottawa - B - really nice place to visit. would probably get a little boring after more than a couple of days though
Atlanta A - had a great time every time I was there.
Tangiers A- - had a guide which made a big difference. went through some interesting (though some seedy) sections without issue. had one of the best meals of my life in a little cafe there.
Malaga, Spain (and towns to the south) A - stayed at a friend's villa overlooking Gibraltar with a view of the African coast. spectacular.
Ruedesheim am Rhein in Germany: stayed in a really nice B&B there on the Rhine. loved it.
Uberlingen (?) Germany: A town on the German side of Lake Constance (I may have actually been on a neighboring town but I think this is the right one)

(Can't believe Providence got a B rating. other than the food, which gets an A, the place bites.)

Been to LA and LV when i was over there when i was younger. Pretty impressed

just to add my experiences to the above....
hong kong, china A+
jakarta, indonesia A -
istanbul, turkey A+
tashkent, uzbekistan B...but depends what you are looking for. quiet and clean and friendly....except the airport. ;)
tripoli, libya C- but in a few years could be an A+, once it is cleaned up and the infrastructure is improved. stunningly beautiful ocean views and the best preserved roman ruins in the world. the old city is right out of the ottoman empire.
beijing, china is C- in terms of pollution, traffic, etc. but A+ in terms of history, sites, etc.
seoul, south korea C since living there is a challenge, what with the air and overpopulation. but since i did live there for a time, i knew all the good places...so for me personally B+ ;D
my experiences in these places is within the past 2 years, so fairly recent.

I personally had the best time in Leuven, Belgium and Cork, Ireland. Those were in my student days, though.
Rome and Montreal rate highly. Dublin is fun, but expensive.
In terms of worst places, South Africa is a beautiful country, but Johannesburg is terrible.
In the US, I'll always say New York, even if it is made up of meathead Yankees fans. ;)

If you're ever over there again, take the train to St. Petersburg. Another A+ city for sure.
